BackgroundWhen patients with Fontan circulation require a computed tomographic pulmonary angiogram (CTPA), there are significant challenges in achieving adequate contrast opacification due to the altered anatomical connections. This study used Time Resolved Angiography with Interleaved Stochastic Trajectories (TWIST) Magnetic Resonance Angiography (MRA) to examine contrast circulation in a cohort of patients with Fontan circulation who were having routine MRI follow up to inform the contrast timing of any subsequent CT.MethodsThis is a single centre, cross-sectional, observational, retrospective study. The time to peak (TTP) signal intensity from the MRA was recorded using regions of interest on the aorta, pulmonary arteries, cavae and Fontan conduit. Patients were grouped by ejection fraction, global longitudinal strain, indexed stroke volume and cardiac index to examine if these cardiac performance parameters affected the mean TTP. Statistical analysis was performed to find the mean TTP for each of the vessels, which was consequently compared between the different cardiac performance parameters.Results35 patients were included in the study. Mean TTP contrast enhancement was 31s in the thoracic aorta, 46s in the right pulmonary artery, 41s in the left pulmonary artery and 55s in the Fontan conduit. Cardiac performance shows no statistically significant relationship to the peak contrast enhancement whether measured by ejection fraction, global longitudinal strain, stroke volume index or cardiac index.ConclusionThe mean optimal timing for a single-phase examination of the Fontan circulation, following an upper limb injection, was 55 s following start of contrast injection irrespective of cardiac performance. In TWIST MRA, the IV bolus is 4–5 s duration. A longer bolus is required for CTA, around 20s, suggesting an additional delay will be required. We propose that an optimal single phase CTPA to be protocolled at 70 s following the start of contrast injection, assuming adequate iodinated contrast dose. 相似文献

目的缩短全口义齿修复患者平均椅旁治疗浪费时间,提高工作质量和效率。方法按照问题解决型品管圈活动步骤进行问题改进,改进措施包括专项学习个别托盘制作、全数字化流程制作支架、改善基础设施、加强培训等。结果全口义齿修复患者平均椅旁治疗浪费时间从113 min缩短至28 min,提高了工作效率,提升了圈员能力。结论品管圈作为一项有效的质量管理工具,其应用可以缩短全口义齿修复患者平均椅旁治疗浪费时间,值得推广。 相似文献

IntroductionDating the exact or estimated time of trauma is an important issue facing forensic medicine. Several clinical and radiological methods were used to achieve this purpose. In the recent study, we aimed to track the changes in the signal intensity of the extra-axial brain hematoma using magnetic resonance imaging (MRI) conventional sequences as well as diffusion-weighted imaging (DWI) and the apparent diffusion coefficient (ADC).Materials and methodsConsidering inclusion and exclusion criteria, all patients with blunt head trauma were involved. After proper management., stabilization, and resuscitation, the participants were assessed using conventional sequences of MRI and DWI twenty-four hours, forty-eight hours, and three weeks after the injury. Temporal changes of signal intensity were compared by Wilcoxon ranged test.ResultsSixteen patients sustaining blunt head trauma were included in this study. The study showed that during the time, diffusion restriction could be seen in an extraaxial hematoma. At the first 24 hours, the signal of hematoma was void in 87.5% of DWI and 100% of ADC. On the second day, they were hypo-signal in 75% of DWI and 100% 0f ADCs, and after three weeks, 100% of cases were hyper-signal in DWI and hypo-signal ADCs.ConclusionThis preliminary study has shown that the DWI can be used to detect and track the extra-axial hematoma. The signal intensity was void during the first twentyfour hours, although it became hypo-signal after 48 hours. Of note, the diffusion restriction is noted after three weeks. 相似文献

ObjectiveValidation of a non-targeted method for urine drug screening (UDS) by liquid chromatography quadrupole time-of-flight mass spectrometry (LC-QTOF), and comparison to an established GC–MS method in a hospital setting.Methods217 UDS specimens sent to a quaternary hospital pathology department, were analysed by a CEDIA® immunoassay screen (six drug panels; amphetamines, barbiturates, benzodiazepines, cocaine metabolites, cannabinoids and opiates) on an Abbott Architect instrument. Specimens were subsequently analysed by an established non-targeted qualitative GC–MS method and results compared with a general unknown screening method by LC-QTOF that was under evaluation as a replacement method.Results42 selected drugs were evaluated; limits of identification ranged from 2 to 100 µg/L and most drugs (n = 39) were stabile for 24 h after preparation. Matrix effects greater than 25% were observed in seven of the selected drugs. 87% of the specimens tested positive to 1 or more drug panels in a CEDIA® screen. A total of 537 positive drug findings were identified by GC–MS compared to 1,267 positive findings by LC-QTOF. On average, each GC–MS screen identified 2.5 ± 1.8 drugs and the LC-QTOF screen identified 5.8 ± 3.2 drugs. No drugs were identified in 11.3% of the GC–MS screens, whereas drugs were detected in 99% of these by the LC-QTOF. In almost all instances, the LC-QTOF screen could provide mass spectrometric confirmatory results of positive immunoassay screens and was able to identify a wider range of additional drugs and drug metabolites.ConclusionsThe described general unknown screening (non-targeted, qualitative) LC-QTOF method can detect a larger range of drugs encountered in a hospital setting. Performing a cognitive task while balancing can result in either increased or decreased sway depending on the nature of the cognitive task, and is commonly used in pathologic populations to evaluate postural performance. A total of 39 participants were recruited into two groups: uninjured controls (n = 20, age: 21.9 ± 2.1 years, height: 175.0 ± 11.2 cm, mass: 71.3 ± 14.9 kg) and chronic ankle instability (n = 19, age: 22.1 ± 5.6 years, height: 169.7 ± 7.7 cm, mass: 72.9 ± 17.3 kg). Participants were asked to perform one of three cognitive tasks while maintaining single limb balance. Cognitive tasks included backwards counting by 3 (BC), the manikin test (MAN), and random number generation (RNG). Time-to-boundary minima, mean, and standard deviations were calculated and compared between groups as pre to post change scores. Effect sizes and 95% confidence intervals were also calculated to test for group differences and the effect of task performance on sway. No significant main effects of Group or Group by Task interactions were identified (p > 0.05). However, a significant multivariate main effect of Task was identified in BC (p = 0.001, F(6, 32) = 4.804) and RNG (p < 0.001, F(6, 32) = 6.233) but not for MAN (p = 0.117). The results suggest that those with chronic ankle instability and uninjured controls have similar postural–suprapostural interactions across multiple cognitive task domains. Both the BC and RNG tasks resulted in less sway for all participants. Our results suggest that dual-task interference in the CAI population may not be present as previous research would suggest. 相似文献
